Pharmacist’s Guide to Supplement–Drug Interactions
Taking a vitamin, mineral or herbal supplement does not automatically mean that it is safe to combine it with your medication.
Some supplements can:
- Reduce the amount of a medicine your body absorbs
- Increase the amount of a medicine in your blood
- Speed up or slow down how the liver breaks down a drug
- Increase side effects
- Increase bleeding risk
- Lower or raise blood pressure or blood sugar
- Change electrolyte levels
- Interfere with laboratory tests
The opposite can also happen: some medicines can reduce the absorption or levels of vitamins and minerals.
The most important message is simple:
“Natural” does not mean “interaction-free.”
The risk is particularly important with medicines that have a narrow therapeutic range, where relatively small changes in drug levels can have significant consequences. Examples include warfarin, digoxin, cyclosporine and some cancer medicines.
The 3 Main Ways Supplements Interact With Medicines
1. They can reduce medication absorption
The supplement and medication may bind together in the intestine, preventing the medication from being absorbed properly.
This is particularly important with:
- Calcium
- Magnesium
- Iron
- Zinc
and certain:
- Antibiotics
- Thyroid medicines
- Osteoporosis medicines
2. They can change how the body breaks down a medicine
Some herbs can change enzymes or transport proteins involved in drug metabolism.
The best-known example is St. John’s wort.
It can increase the activity of certain drug-metabolizing systems and reduce the effectiveness of many medicines.
3. They can increase the effects or side effects of a medicine
For example:
- Omega-3 supplements may have antiplatelet effects.
- Garlic supplements may increase bleeding risk.
- Vitamin E can increase bleeding risk at high supplemental doses.
- Potassium supplements can cause dangerously high potassium when combined with certain medicines.
- Some supplements can lower blood pressure or blood glucose and potentially add to the effect of medication.

These are examples, not a complete interaction database. The exact medication, dose, formulation, timing and patient’s health conditions matter.
Supplements and Blood Thinners
This is one of the most important areas of supplement safety.
“Blood thinner” is a general term that includes different types of medicines. They do not all work in the same way, so an interaction with warfarin cannot automatically be applied to every anticoagulant.
Vitamin K + Warfarin
Vitamin K is essential for normal blood clotting.
Warfarin works by interfering with vitamin K-dependent clotting-factor activation.
Increasing vitamin K intake substantially can therefore reduce warfarin’s anticoagulant effect.
But suddenly reducing vitamin K intake can also affect anticoagulation.
The goal is consistency.
If you take warfarin:
- Do not suddenly start a vitamin K supplement.
- Do not suddenly stop one.
- Do not dramatically change your intake of vitamin-K-rich foods.
- Tell your healthcare professional about supplements containing vitamin K.
The important issue is not that vitamin K is dangerous for healthy people. The important issue is that it can interfere with vitamin K-antagonist anticoagulants.
Omega-3 + Anticoagulants
Fish oil and other omega-3 supplements can have antiplatelet effects, particularly at higher doses.
Clinical studies have generally not shown a major increase in clinically significant bleeding with commonly used doses, but omega-3 products can affect INR in some people taking warfarin, and monitoring may be appropriate when higher doses are used.
Practical advice
If you take:
- Warfarin
- Another anticoagulant
- Antiplatelet medication
tell your doctor or pharmacist before starting high-dose omega-3.
Do not stop prescribed anticoagulation because you are taking fish oil.
Ginkgo + Blood Thinners
Ginkgo biloba is commonly marketed for memory and circulation.
However, ginkgo may increase bleeding risk when taken with anticoagulants such as warfarin.
Be particularly cautious if you take:
- Warfarin
- Aspirin
- Clopidogrel
- Other anticoagulant or antiplatelet medicines
Garlic Supplements + Blood Thinners
Garlic in normal food amounts is very different from concentrated garlic supplements.
Garlic supplements may increase bleeding risk, particularly when combined with anticoagulants or aspirin.
This is especially important before surgery.
Vitamin E + Blood Thinners
High-dose vitamin E supplements can inhibit platelet aggregation and interfere with vitamin K-dependent clotting.
Large supplemental doses may therefore increase bleeding risk, particularly when combined with anticoagulants or antiplatelet medicines.
Important:
This does not mean that vitamin E in normal foods is dangerous.
The concern is mainly with supplemental doses, particularly high doses.
Thyroid Medication and Supplements
Levothyroxine + Calcium
Calcium carbonate can reduce absorption of levothyroxine.
The FDA-approved labeling recommends separating levothyroxine and calcium carbonate by at least 4 hours.
Simple example:
Morning: levothyroxine
Later in the day: calcium
Do not assume that taking them together is harmless simply because calcium is a nutrient.
Levothyroxine + Iron
Iron can also reduce levothyroxine absorption.
Product labeling recommends separating levothyroxine from iron supplements by at least 4 hours.
Therefore:
If you take both thyroid medication and iron, don’t take them together unless your prescriber has specifically instructed you to do so.
Thyroid Medication + Magnesium
Because magnesium can interfere with the absorption of some medicines, it is sensible to check the specific product information when taking magnesium with levothyroxine or other thyroid medicines.
The strongest established timing interactions are with calcium and iron, so those should receive particular attention.
Antibiotics + Minerals
This is one of the most common mistakes people make.
Several minerals can bind to certain antibiotics in the intestine.
This can reduce absorption of the antibiotic.
Magnesium + Antibiotics
Magnesium can bind tetracycline antibiotics such as doxycycline and quinolone antibiotics such as ciprofloxacin and levofloxacin.
A common approach is to take the antibiotic at least 2 hours before or 4–6 hours after magnesium, but the exact instructions should follow the specific medication label or pharmacist’s advice.
Zinc + Antibiotics
Zinc can interfere with absorption of:
- Tetracycline antibiotics
- Quinolone antibiotics
The antibiotic should generally be taken at least 2 hours before or 4–6 hours after zinc.
Calcium + Antibiotics
Calcium can reduce absorption of certain quinolone antibiotics.
For example, calcium should be separated from medications such as ciprofloxacin according to the specific medication instructions.
Easy rule:
Do not take calcium, magnesium, iron or zinc at the same time as an antibiotic unless you have checked that specific combination.
Iron + Levodopa
Iron can reduce absorption of levodopa, a medicine used in Parkinson’s disease and some other conditions.
Iron supplements should therefore be separated from levodopa according to the medication instructions.
Iron + Calcium
Calcium can interfere with iron absorption.
If you take both supplements, taking them at different times of the day may help avoid this problem.
This is especially relevant when someone takes:
Iron + calcium + magnesium + zinc + multivitamin
all at the same time.
You may be taking several nutrients that compete with or interfere with medication or mineral absorption.
Osteoporosis Medicines + Minerals
Oral bisphosphonates such as alendronate are particularly sensitive to absorption problems.
Magnesium-containing supplements or medicines can reduce absorption of oral bisphosphonates.
They should be separated by at least 2 hours, and the medication itself often has very specific administration instructions.
Important:
If you take an osteoporosis medicine, follow its instructions before adding calcium, magnesium or other minerals.
Blood Pressure Medicines + Supplements
Some supplements can affect blood pressure.
This may become a problem when they are combined with antihypertensive medication.
Examples include:
- Omega-3 supplements
- Magnesium in some circumstances
- Garlic supplements
- Certain herbal products
The effect varies considerably by product and dose.
Don’t assume:
“It lowers blood pressure naturally, so combining it with my medicine is better.”
Too much blood-pressure lowering can cause:
- Dizziness
- Weakness
- Falls
- Fainting
Potassium Supplements: A Special Warning
Potassium deserves special attention.
Some medications reduce potassium excretion and can cause hyperkalemia, especially in people with kidney disease.
Important examples include:
- ACE inhibitors
- ARBs
- Spironolactone
- Amiloride
- Some other potassium-sparing medicines
Combining these medicines with potassium supplements or potassium-containing salt substitutes can raise potassium too much.
High potassium can be dangerous.
Severe hyperkalemia can cause abnormal heart rhythms.
Therefore:
Do not start potassium supplements simply because you think you might be low in potassium.
A blood test and medical assessment may be appropriate.
Vitamin D + Medications
Vitamin D is generally well tolerated at appropriate doses, but it can interact with certain medicines.
Vitamin D + Thiazide Diuretics
Thiazide diuretics reduce urinary calcium loss.
When combined with vitamin D, this can contribute to high blood calcium, particularly in older adults, people with impaired kidney function or hyperparathyroidism.
Vitamin D + Orlistat
Orlistat reduces fat absorption and can reduce absorption of vitamin D.
Vitamin D + Corticosteroids
Long-term corticosteroids such as prednisone can reduce calcium absorption and interfere with vitamin D metabolism.
This is one reason bone health deserves attention in people receiving prolonged steroid therapy.
Iodine: Supplements, Kelp, Topical Iodine & Medication Interactions
Iodine is an essential trace mineral required to make the thyroid hormones T4 (thyroxine) and T3 (triiodothyronine).
These hormones influence metabolism, energy production, growth and development. Iodine is particularly important during pregnancy and early childhood because thyroid hormones are essential for normal brain and nervous-system development.
Most people should aim to obtain adequate iodine rather than taking large amounts “just in case.”
Recommended daily intake
For adults, the recommended intake is approximately 150 mcg per day.
Requirements increase to:
- 220 mcg/day during pregnancy
- 290 mcg/day during breastfeeding
Iodine Supplements
Iodine supplements commonly contain:
- Potassium iodide
- Sodium iodide
- Kelp or other seaweed extracts
Many multivitamins also contain iodine, commonly around 150 mcg per serving.
More iodine is NOT necessarily better.
The adult tolerable upper intake level is 1,100 mcg (1.1 mg) per day from all sources.
Excessive iodine can cause some of the same problems as iodine deficiency, including goiter and hypothyroidism, and can also trigger hyperthyroidism or thyroid inflammation in susceptible people.
People with existing thyroid disease may be particularly sensitive to excessive iodine.
Be careful with high-dose iodine products
Some supplements marketed as “high potency iodine” contain iodine in milligram quantities, far above the normal nutritional requirement.
For example, a product containing several milligrams of iodine can provide many times the adult recommended intake and exceed the established upper limit.
Do not take high-dose iodine supplements simply to “support the thyroid” unless there is a specific medical reason and professional guidance.
Iodine and Thyroid Medication
Iodine deserves special attention if you have thyroid disease or take thyroid medication.
Antithyroid medicines
High-dose iodine can interact with antithyroid medicines such as methimazole, potentially contributing to excessive suppression of thyroid hormone production.
If you take medication for hyperthyroidism, do not start an iodine supplement without discussing it with your doctor or pharmacist.
Levothyroxine
The interaction is different with levothyroxine.
Iodine is needed to make thyroid hormones, but taking extra iodine is not a substitute for levothyroxine and should not be used to adjust thyroid function on your own.
If you take levothyroxine, particularly if you have Hashimoto’s disease or another thyroid disorder, discuss iodine supplementation with your healthcare professional before adding it.
Iodine + ACE Inhibitors
This interaction is easy to miss.
Some iodine supplements contain potassium iodide.
Potassium iodide can contribute potassium, and combining it with medicines that raise potassium levels can increase the risk of hyperkalemia.
This is particularly relevant with ACE inhibitors, such as:
- Lisinopril
- Enalapril
- Benazepril
- Fosinopril
The NIH specifically warns that potassium iodide combined with ACE inhibitors can increase blood potassium levels.
Important:
The concern is particularly with potassium iodide, not simply with the word “iodine.”
Check the actual ingredient and dose on the supplement label.
Iodine + Potassium-Sparing Diuretics
The same issue applies to medicines that reduce potassium excretion.
Examples include:
- Spironolactone
- Amiloride
Combining potassium iodide with these medicines can increase the risk of excessively high potassium levels.
People taking these medicines should not start potassium iodide supplements without professional advice.
Kelp and Seaweed Supplements: Don’t Assume They Are Mild
Kelp supplements can contain iodine, sometimes in highly variable amounts.
This is important because the iodine content of seaweed can vary considerably.
Therefore:
“It’s just kelp” does not necessarily mean that the iodine dose is small or predictable.
If you already take a multivitamin containing iodine, adding a kelp supplement can substantially increase your total iodine intake.
What About Topical Iodine?
Topical iodine is different from oral iodine supplements.
Products containing iodine are used on the skin as antiseptics, particularly preparations such as povidone-iodine.
They are intended primarily for external use, not as nutritional iodine supplements.
Do not drink topical iodine.
Solutions intended for skin application should not be used as oral iodine supplements unless a product is specifically formulated and approved for oral use.
Can Topical Iodine Affect the Thyroid?
Usually, normal short-term topical use does not cause a problem in healthy adults.
However, iodine can be absorbed through the skin, and absorption can be greater with:
- Large areas of application
- Repeated or prolonged use
- Damaged skin
- Burns
- Occlusive dressings
- Certain medical situations
- Infants and very young children
Significant iodine exposure can affect thyroid function in susceptible individuals.
Therefore, prolonged or extensive use of iodine-containing antiseptics should not be treated as completely irrelevant to total iodine exposure.
Topical Iodine Before Thyroid Testing or Treatment
Tell your doctor if you have recently used iodine-containing products, particularly if you are undergoing:
- Thyroid imaging
- Radioactive iodine testing
- Radioactive iodine treatment
- Evaluation for thyroid disease
Iodine exposure can affect some thyroid-related procedures and preparations. Patients undergoing radioactive iodine procedures may be specifically instructed to follow a low-iodine diet and avoid certain iodine-containing products.
Iodine and Medical Imaging Contrast
Iodine-containing contrast agents used for some CT and other imaging procedures are not the same thing as iodine supplements.
However, they can provide a substantial iodine exposure.
If you are being evaluated for thyroid disease or preparing for radioactive iodine treatment, tell your healthcare team about any recent imaging involving iodinated contrast.

Can You Get Too Much Iodine?
Yes.
Unlike some nutrients where more may provide additional benefit, excess iodine can disturb thyroid function.
For adults, the established upper limit is 1,100 mcg/day from food and supplements.
Symptoms and effects of excessive iodine can include:
- Goiter
- Hypothyroidism
- Hyperthyroidism
- Thyroid inflammation
The risk is not identical for everyone. Some people, particularly those with thyroid disease or previous iodine deficiency, may be more sensitive to excessive intake.
Bottom line:
Do not use high-dose iodine as a general “thyroid booster.”
Iodine: The Simple Rule
If you are healthy:
Aim for adequate iodine, preferably from food and appropriately formulated supplements when needed.
If you have thyroid disease:
Don’t start high-dose iodine without professional advice.
If you take methimazole or another antithyroid medicine:
Check with your doctor before taking iodine supplements.
If you take lisinopril, another ACE inhibitor, spironolactone or amiloride:
Check whether an iodine product contains potassium iodide before using it.
If you use topical iodine:
Use it as directed for external application and don’t treat topical iodine as a nutritional supplement.
If you are preparing for radioactive iodine testing or treatment:
Tell your medical team about iodine supplements, kelp products, iodine-containing antiseptics and recent iodinated contrast exposure.
Zinc + Medications
Zinc is useful nutritionally, but higher doses are not automatically better.
Important interactions include:
Zinc + antibiotics
Can reduce absorption of both zinc and certain antibiotics.
Zinc + penicillamine
Zinc can reduce absorption and effectiveness of penicillamine.
Zinc + thiazide diuretics
Long-term thiazide use can increase urinary zinc loss.
Magnesium + Medications
Magnesium is one of the supplements that most commonly creates timing issues.
Important examples include:
Magnesium + antibiotics
Can reduce absorption of tetracyclines and quinolones.
Magnesium + oral bisphosphonates
Can reduce absorption.
Magnesium + diuretics
Some diuretics increase magnesium loss; others can reduce it.
Magnesium + proton-pump inhibitors
Long-term PPI therapy can contribute to low magnesium in some people.
Vitamin C + Medicines
Vitamin C is generally considered safe at nutritional doses, but high-dose supplementation deserves caution.
Cancer treatment
People receiving chemotherapy or radiation should discuss vitamin C and other antioxidant supplements with their oncologist.
The evidence about whether antioxidants interfere with cancer treatment is complicated and varies according to treatment and dose, so don’t make the decision yourself based on the assumption that antioxidants are always helpful or always harmful.
Hemochromatosis
High-dose vitamin C can worsen iron overload in people with hereditary hemochromatosis.
Vitamin E + Medicines
Vitamin E deserves caution at high supplemental doses.
Potential concerns include:
- Anticoagulants
- Antiplatelet medicines
- Aspirin
- Cancer treatment
High doses can increase bleeding risk, and antioxidant supplementation may potentially interfere with some cancer treatments.
Biotin: The Interaction Most People Don’t Know About
Biotin is often found in:
- Hair supplements
- Nail supplements
- Multivitamins
- “Beauty” formulas
It is usually well tolerated, but there is a completely different safety issue:
Biotin can interfere with laboratory tests.
High doses can produce misleading results in certain tests, including some thyroid tests and troponin tests used when evaluating possible heart attacks.
The FDA has specifically warned that biotin interference can cause falsely low troponin results in certain assays, potentially creating a dangerous false impression that a heart attack has not occurred.
Always tell your doctor and laboratory:
“I take biotin.”
Especially before blood testing.
Herbal Supplements: The Rules Are Different
Herbal supplements can be much harder to evaluate than vitamins and minerals.
Why?
Because an herb can contain many active compounds, and different products may contain different extracts, concentrations and formulations.
NCCIH notes that for many herbs there is simply not enough reliable human evidence to know whether a clinically important interaction exists.
But several herbs have well-established or important interaction concerns.
St. John’s Wort: The Biggest Interaction Problem
If there is one herbal supplement that readers should remember, it is:
St. John’s wort
It can interact with a very large number of medicines.
It can reduce the effectiveness of medicines including:
- Some antidepressants
- Birth-control pills
- Warfarin
- Cyclosporine
- Digoxin
- Some HIV medicines
- Some cancer medicines
- Some statins
- Certain seizure medicines
- Other drugs metabolized by affected pathways
It can also interact with serotonergic medicines and increase the risk of serious serotonin-related adverse effects.
Our advice:
If you take prescription medication, do not start St. John’s wort without checking with your doctor or pharmacist.
Ginkgo Biloba
Ginkgo is commonly marketed for memory and circulation.
Potential concerns include:
- Increased bleeding risk with warfarin
- Possible interactions with other medications
- Additional uncertainty because supplement preparations vary.
Asian Ginseng
Asian ginseng may interact with some:
- Blood-pressure medicines
- Calcium-channel blockers
- Statins
- Antidepressants
Evidence concerning interaction with warfarin has been mixed.
Ginseng may also lower blood glucose and may interfere with blood clotting.
If you take diabetes medication:
Discuss ginseng with your healthcare professional before using it.
Garlic Supplements
Garlic supplements can potentially increase bleeding risk, particularly with:
- Warfarin
- Aspirin
- Other anticoagulants or antiplatelet drugs

Green Tea Extract
Green tea as a beverage is not equivalent to concentrated green tea extract.
NCCIH notes that concentrated green tea supplements can reduce blood levels of some medicines, including atorvastatin and nadolol.
This is an excellent example of why:
“I drink green tea” and “I take a concentrated green tea extract” are not necessarily the same exposure.
Goldenseal
Goldenseal may change the way the body processes several medications.
NCCIH specifically notes evidence that goldenseal extract can lower levels of metformin.
People with diabetes should therefore be particularly cautious about using goldenseal.
Echinacea
Echinacea is widely used in immune-support products.
Unlike St. John’s wort, it does not have the same broad, well-established interaction profile.
However, evidence for many possible herb-drug interactions is limited, and individual products differ.
If you take prescription medication regularly, particularly medicines with a narrow therapeutic range or medicines affecting the immune system, ask a pharmacist before combining them.
This is a good example of where we should say:
“Evidence is limited”
rather than either:
❌ “Echinacea is completely safe with medication.”
or:
❌ “Echinacea interacts with everything.”
Ashwagandha
Ashwagandha is increasingly popular for stress, sleep and other purposes.
However, it should not be treated as an interaction-free herb.
Potential concerns include:
- Sedative effects
- Effects on thyroid function
- Possible effects on blood glucose
- Possible effects on blood pressure
- Interactions with certain medications
Because evidence is still developing, people taking multiple medications should discuss ashwagandha with a healthcare professional before using it regularly.
Propolis
Propolis is another popular natural product, especially in immune-support and throat preparations.
The interaction evidence is less extensive than for St. John’s wort.
However, people with bee-product allergies should be cautious, and concentrated supplements should not automatically be assumed to be harmless simply because propolis is natural.
If you take prescription medication or use propolis regularly in high doses, check the specific product with your pharmacist.
CBD and Other Cannabinoid Products
CBD deserves special attention because it can affect the metabolism of certain medicines.
It may interact with medicines that are metabolized through the same enzyme systems.
This can potentially increase or decrease medication levels.
NCCIH specifically lists CBD among supplements/products that may interact with many medications.
Don’t combine CBD with prescription medicines casually.
Supplements and Diabetes Medicines
Supplements can potentially lower blood glucose or change the way diabetes medicines work.
This is particularly relevant to products marketed for:
- Blood sugar control
- Weight loss
- Metabolic health
Examples include:
- Berberine
- Ginseng
- Some herbal blends
- High-dose chromium
- Some other botanicals
Important:
If you take insulin or other glucose-lowering medication, adding a supplement that may also lower glucose can potentially increase the risk of hypoglycemia.
Don’t use a supplement as a reason to reduce or stop prescribed diabetes medication.
Supplements and Antidepressants
This is another area where people need to be careful.
The best-known example is:
St. John’s wort + antidepressants
Combining St. John’s wort with medicines that affect serotonin can increase the risk of excessive serotonin activity and potentially serious adverse effects.
Don’t assume:
“It’s natural, so it can be combined with my antidepressant.”
It cannot.
Supplements and Cancer Treatment
Cancer patients should be particularly careful with supplements.
Some antioxidants, herbs and other supplements may affect:
- Drug metabolism
- Drug absorption
- Oxidative mechanisms
- Treatment effectiveness
- Side effects

Laxatives and Supplements/Medicines

Senna and stimulant laxatives
Senna is a stimulant laxative used for short-term relief of constipation.
Long-term or excessive use can contribute to diarrhea, dehydration and electrolyte disturbances, particularly low potassium. People who repeatedly need laxatives should discuss the underlying cause of constipation with a healthcare professional rather than simply increasing the dose. NIDDK recommends that stimulant laxatives generally be reserved for severe constipation or when other treatments have not worked.
This becomes particularly important when senna is combined with medicines that already affect potassium or fluid balance, including some diuretics and certain heart or blood-pressure medicines.
Do not routinely combine stimulant laxatives with potassium supplements to “replace what was lost” without medical advice. The correct treatment depends on the actual cause and severity of any electrolyte abnormality.
Mineral laxatives
Some laxatives contain magnesium.
This is important for people with kidney disease, because impaired kidneys may have difficulty eliminating excess magnesium.
Magnesium-containing laxatives can also contribute to the total magnesium intake from supplements and medicines.
Calcium-containing antacids and constipation
Calcium-containing antacids can themselves contribute to constipation. NIDDK lists calcium- and aluminum-containing antacids, iron supplements, some antidepressants, calcium-channel blockers, opioid pain medicines and several other medicines among possible causes of constipation.
A simple rule
If you regularly need a laxative, don’t just keep increasing the dose. Find out why you are constipated.
Constipation can be caused by diet, dehydration, inactivity, medications, supplements and medical conditions. Persistent constipation or warning signs such as blood in the stool, vomiting, unexplained weight loss, severe abdominal pain or inability to pass gas require medical evaluation.
